For Office Procedures & New Patient Consultations
We at Dr. Park Avenue take pride in the appropriate reservation of your procedural date and time. Our priority is to schedule procedures that can be attended to with the utmost of quality and care. We strive to ensure that each patient is met with the same standard of care and quality of service.
Policy for non-surgical procedures:
- Payments and/or deposits for certain non-surgical procedures and consultations will be taken at the time of scheduling to secure your appointment
- Cancellations 48 hours prior to your procedure(s) will result in a charge to your account: $100 for Smart Lipo consultations and $50 for all other services.
- After 1 occurrence of an appointment no-show, all balances must be paid prior to scheduling any future appointments
Policy for new patient consultations:
- Please arrive 15 minutes prior to your initial appointment to ensure that you are seen in a timely manner
- Arriving 15 minutes or more late to your appointment will result in forfeit of your scheduled appointment time
